If you would like to file an Investigation Request with the Safe Sites and Structures Board, please print or download the form in the menu to the left of this page.  Investigation Requests must be signed by the complainant, or they will not be considered.  The completed paperwork can either be mailed, emailed or delivered to Greg Harris at the addresses above.  The Safe Sites and Structures Board meets on the 2nd Thursday of each month at 3:00 pm at the Upshur County Administrative Annex, 91 West Main Street, Suite 101.  The Safe Sites and Structures Board does not regulate household garbage.  If you have a complaint related to household garbage, please call the DNR at (304) 924-6211.
